Payment, Reschedule and Cancellation Polices

Flourish Feeding Babies LLC accepts payments at the time that services are provided. These will be automatically charged to the card on file as per the authorization at booking. A discount will be provided for follow up visits and is reflected in the Self-pay costs listed on our website.

A surcharge will be applied to home visits outside my service area. If parking fees are incurred, these will also be billed separately and on the day of the appointment. These surcharges are not eligible for insurance reimbursement.

A surcharge will be applied if you have twins or higher order multiples to cover the extra length of the visit. This can be submitted to insurance via superbill.

Cancellation or reschedule is allowed up to 24 hours prior to an appointment for a refund. If appointments are cancelled or rescheduled with less than 24 hours notice, a $150 fee will be charged to the credit card provided. All appointments cancelled within one hour or less of the appointment time will be charged in full of self-care rates. These cancellation surcharges are not eligible for insurance reimbursement. This cancellation policy also applies to patients who have coverage approved through TLN.

Aetna and AmerihealthNJ: Deductibles and co-insurance charges may apply. It is possible that some insurance plans, even if they state that you are fully covered, may assign a ‘Patient Responsibility’ , co-insurance, or "Deductible" which will be charged to the card on file. If your baby is not on the same Aetna plan as you, a $75 charge will apply and is charged directly to the card on file.

If the insurance supplied on the intake is not your primary plan and I am not in-network with your primary plan, you will be charged self-pay rates directly on your card on file.
